A surrogate checklist helps you assess your readiness for surrogacy. It covers health, support, expectations, and practical considerations before matching.

Readiness depends on your health, pregnancy history, support system, and whether you meet the basic requirements for surrogacy. Take our checklist to review the main requirements, or see our Surrogate Requirements guide for more details.

Beyond eligibility, consider the physical and emotional commitment, the time involved, how surrogacy may affect your family, and the relationship you want with the intended parents.

Surrogate disqualifications usually fall into three areas: medical history (age outside 21–43, BMI outside roughly 18–33), lifestyle (current smoking or drug use), and legal or financial factors (living where surrogacy is restricted). Many are temporary, so one concern doesn't automatically rule you out.

Screening usually includes medical clearance, a psychological evaluation, background checks, and legal review. The process typically takes two to four months after you match with intended parents.
